Job Summary:

Healthier Moms and Babies is accepting applications for the position of a Case Manager. Within Healthier Moms and Babies and reporting to the HMB Supervisor, the Case Manager provides health education and case management services to low-income, high-risk pregnant women in Allen County, visiting them in their homes from as early in pregnancy as possible, with a goal of improving the outcome of pregnancies and reducing infant mortality.



Minimum of a Bachelor’s degree in Human Behavioral Science, Social Work or Human Services. Education focus or education minor on drug addiction, alcohol addiction, education, counseling, psychology, child development, family intervention, therapeutic techniques, preferred. Must have current valid Driver’s License. Minimum of two years’ experience required. Case Management, preferably with home visits, experience with maternal child health; familiarity with local social service delivery network preferred. Previous experience in case management, home health, maternal child health or other work directly related to the mission of Healthier Moms and Babies preferred. Knowledge of pregnancy, labor and delivery, infancy, of community agencies and social service network, ability to work with diverse cultures and a willingness to make home visits. Demonstrates integrity in all situations and interactions; has the ability to develop trusting, collaborative, professional relationships with coworkers, clients, and community collaborators. Able to communicate clearly, concisely, and in a tactful, professional manner. Demonstrates initiative, self-motivation, and pro-active behavior. Demonstrates organizational skills, performs accurate work and shares accurate information relevant to the organization. Must have superior listening skills, written and verbal communication skills, and presentation skills. Must have superior conflict resolution and/or mediation skills. Computer skills Microsoft Outlook, Office, and Excel required. Experience with and knowledge of Medicaid preferred.


Work Hours:


Monday-Friday 8:00 a.m. - 5:00 p.m.

Hours may vary based upon client needs
