Healthier Mom's & Babies Nurse Family Partnership ~ Nurse Home Visitor

Applications are being accepted for the position of Healthier Moms and Babies (HMB) Nurse Family Partnership (NFP) Nurse Home Visitor.  FWMEP is accepting applications from internal and external applicants.

Job Summary:

Within Healthier Moms and Babies and reporting to the NFP Supervisor, the Nurse Home Visitor is responsible for maintaining the highest standards in clinical nursing practice and adherence to the NFP model.  The Nurse Home Visitor is also responsible for following policies, procedures, guidelines and standards of NFP, and for Healthier Moms and Babies.

Nurse-Family Partnership, a maternal and early childhood home visiting program, fosters long-term success for first-time moms, their babies, and society.  Nurse-Family Partnership's maternal health program introduces vulnerable first-time parents to caring maternal and child health nurses.  This program allows nurses to deliver the support first-time moms need to have a healthy pregnancy, become knowledgeable and responsible parents, and provide their babies with the best possible start in life.

BSN Required. Current RN Indiana State License (in good standing) and Current CPR required.  Two years of recent experience in maternal child health, public health, mental/behavioral nursing preferred.  Home visiting preferred.  Current or recent public/community health experience highly preferred.  Must be able to work a flexible schedule including some evenings and weekends.  Candidate must demonstrate a passion for excellence and a commitment to lifelong learning.  Additionally, the candidate must possess knowledge of community agencies and social service network.  The candidate must have the ability to work with diverse cultures and a willingness to make home visits.  The candidate must demonstrate team player abilities while striving for excellence in patient care.  Computer literacy in Microsoft Outlook, Office, and Excel required.

Work Hours:

Monday-Friday; 8:00 a.m. - 5:00 p.m.
